Task:
Overview of the assignment task
1.For this assignment, you will be presented with 2 detailed case studies.
2.Each case study will be followed by a series of questions which you will need to answer.
3.The questions are designed to assess your clinical approach/reasoning from a clinical examination perspective. The clinical framework used for this assignment is based on the clinical reasoning approach you were taught in session 8, and again in session 13.
4.For this assignment, you will be required to explain/justify your clinical decisions based on the presentation of the client, relevant signs and symptoms, and your developing differential diagnosis.
5.Whilst this is not a research-based assignment, any references you use in preparation of your assignment must be cited using the APA referencing style.
6.The focus of this assignment is on your assessment/evaluation of the client from a clinical examination perspective. You should not discuss treatment aims or treatment plans for the clients presented.
Case Study 1:
Gary is a 58-year-old man who presents with recent onset of excessive flatulence, and mild abdominal pain, which at times may increase to moderate discomfort. Intermittently he has also noted some red blood in his stool which he reports he has had previously when he was diagnosed with early diverticula disease. He is clearly overweight and reports that his doctor is always on at him about the need to lose weight, which would also help his hypertension. Gary reports that he has recently lost a small amount of weight (approx. 3kg), which he attributes to the mild nausea he feels, and has put him off his food. Gary thinks that the nausea may be associated with the Celecoxib (Celebrex) that the doctor has started him on to manage his lower back pain, which has been bothering him more than usual lately. Garys intention for coming to see you was to get some diet and exercise advice to help with his weight and upset gut, but also because his recent blood tests showed that his cholesterol levels were continuing to increase, as were his blood sugar levels.

3.For the priority examination that you have selected in question 2, using the correct order of the examination, describe the findings that you would expect to obtain while performing this examination, briefly explaining the differential diagnosis you are considering.
4.Select 3 additional examinations you will perform on Gary to assess his presentation. Briefly explain why you chose the examination using the presenting signs and symptoms, and briefly explain what you would expect to find from the examination. Case Study 2:
Alison is a 52 year old female who presents for assistance with diet and lifestyle advice to help manage her weight and bone health now that she is post-menopausal. Whilst Alison doesnt consider herself to be overweight, she has noticed a small amount of weight gain (approx. 1-2kg) over the last 12 months. She has a history of fibromyalgia diagnosed 8 years ago, which causes her mild to moderate pain on a daily basis, especially in her hands and feet, and at times can become painful enough to limit her activity, although generally movement will improve her symptoms unless they are severe. She has tried going for morning walks before work, but reports that usually her pain and stiffness is worse in the morning. Currently she is taking Amitriptyline (Elavil) for the fibromyalgia, and advises that she has been taking an aspirin each day to help both with pain, and because she has heard it is good for thinning the blood and preventing heart attacks, which her father died from 4 years earlier. Alison also feels that she has become intolerant of eating fatty foods, because when she does, she starts to burp a lot a short while after the meal. She reports that she has recently developed some reflux after she eats which makes her feel a little unwell, but if she doesnt eat she gets a strong pain in her stomach. Most recently she has also had an irritated throat and minor, but persistent dry cough, which she thinks is probably just a bit of a cold.
